At Inclusion Sports Performance Training we believe in finding the right fit for every child as each student comes with different needs and abilities. Getting yourself set up, allows us to see what we can provide so your child will receive the best overall experience.

Getting set up

Follow the steps below:

  • Complete the intake form and send to us for review at this email address. We will get back to you on how you should proceed

    After we review the intake form and get back to you, please complete the following:

  • Email us the IEP, completed intake form, as well as your name and contact information. One of our staff will get back to you in a timely manor to schedule an evaluation

  • Register as a new student / family

  • email inclusionsportspt@gmail.com to schedule your evaluation indicating your availability.

Please specify if you want a swim evaluation, a gymnastics evaluation or a learn to ride evaluation. Once we have seen a student in either of these evaluations, we can quickly determine the appropriate class for the other program you are looking to do. Most times we don’t need to do an evaluation for both to determine appropriate placement for your child, however, if we find that an additional assessment is needed for a separate program, you will only be billed for the evaluation and not another registration fee.

The fee is $25
The evaluation can take 15-30 min depending on the student
Once you enroll, the annual registration fee is $40. ($60 for families with multiple children enrolled)
